| In your Get Started package, there is a simple form for you to fill out to give us the basic information about you and your medical providers. You also sign an authorization form that allows us to contact your medical providers to collect copies of your records. We then have our customer service professionals contact your doctors and medical facilities, and they will send us copies of your records.which we then load into your own Personal Health Record |

| Due to the relationships we have with many of the healthcare providers in the country, there are times we are able to get copies without incurring additional charges. If there is a charge, however, it is covered in the cost of the service, up to $40 worth of charges. If we reach the $40 maximum, we will contact you to determine if you want to pay for additional record retrieval. There are never any hidden fees with our service. |

| Yes! We have a subscription service that allows you to "refresh" your personal health record with additional information. This will be further described in your "Get Started" package. |

| We keep a copy of your information secured in our data center. If you should lose your key, we can help you get it reloaded into your PHR. |
